*The Affidavit must be mailed and must contain a notary signature and seal.
**The Apply Texas Application can be faxed or mailed.
*** Core Residency Questionnaire and HB 3826 Exemptions can be submitted electronically (To submit electronically, fill in the highlighted fields, create a digital signature and "sign" the document.
