Application for Graduation and Tuition Rebate Form: Complete this form and return to the dean of your college by the deadlines posted on the Academic Affairs website. If you choose to fax the forms to your Dean's Office be sure to followup that it has indeed been received by them.

Academic Suspension Reinstatement Form: Complete this form with the assistance of your academic advisor or the Student Advising, Registration and Retention (STARR) Center. When completed, contact your Academic Dean to schedule a time to discuss the reinstatement.
