Registrar Proof of Enrollment

Proof of Enrollment

You can request enrollment verification at the Office of the Registrar, by mail or fax.  If you don't have a form from the inquiring company, provide the following information:

  • Full name
  • Social security number
  • Current address and phone number
  • Anticipated graduation date
  • Type of verification
  • Indicate semester, academic year or history
  • Verification for (insurance, loan company, bank, scholarship, etc.)?
  • Recipient name
  • Recipient address/fax
  • Signature
